How to Fill in Waivers

With any physical activity program there is an element of risk. We need to make you aware of and have you acknowledge that you understand these risks. This form releases the University of Calgary from any liability for unintended issues that may arise as a result of your program or activity due to circumstances outside our control.

1. Click Login in the top right corner of the page.
2. Login using your email username. If you do not yet have an email username then please login with your UCID number. This is located next to your name on the confirmation of your registration.  If this is your first time logging in, enter your ID number and the temporary password that was emailed to you. You will be prompted to create a password.
3. Click on your name in the top right corner of the page, and then click on Waivers and Forms from the dropdown menu.  Any required waivers will be listed in the Outstanding Waivers section, and can be completed by clicking Sign.

To safely offer programs to you and your family we need to be aware of any medical issues that you may have, as well as any life-threatening conditions including but not limited to severe allergies. We also need to know who to call in case of an emergency and contact information for your family physician.

3. Click on your name in the top right corner of the page, and then click on Waivers and Forms from the dropdown menu. Any required Medical Forms will be listed in the Medical Forms section, and can be added or updated.

The Photo Release is strictly optional for you to fill out.
