Personal Training Client Forms
Prior to getting started with your first session, you will be requested to complete the forms listed below to ensure that we can provide you with the best possible service and training experience.
Save a copy of the form and fill in the fields, then attach it to an email to the program coordinator. Please note, in order to use the electronic copy of this form, you must have Adobe Reader version 5 or greater. Adobe Reader can be downloaded for free from http://get.adobe.com/reader/
If you have difficulty with completing the electronic form or would prefer to submit a paper copy, please download, print, and complete each of the forms
Completed forms can be faxed to 403.289.9117, scanned and emailed to firstname.lastname@example.org or dropped off at the Health and Fitness Programs and Services Office (Kinesiology Building KNB 135) at the University of Calgary.

Rehabilitation and Fitness Package
This program incorporates individually designed programs performed in a group setting. The program runs twice a week for 14 weeks. All programs are specifically designed to meet the individual's goals, with elements of strength training, cardiovascular fitness, flexibility, balance and coordination.
You must be 16 years of age and have a physical disability to participate in this class. It is recommended that first time participants attend an orientation prior to registration.
